Findings: The Licensing Specialist noted that the provider had a loose fitting sheet on one of the Pack 'N Play mats. The provider replaced the original Pack 'N Play with another Pack 'N Play (from storage) which had a larger mat. Now the Pack 'N Play sheet cannot be easily lifted up from the center of the mat. Going forward please make sure that the Pack 'N Play sheets fit snugly to ensure the safety of all children while they are resting.

When the parent completes the parent portion of the health inventory, then the individual child information will be completed too. Please forward written corrective action within 30 days.
