The provider is supposed to have a barrier to the laundry room due to hazards on the laundry machines and the dog using puppy pads in that room. It has been discussed with the provider to put in a barrier that allows the dog to use the room but not the children to get in. Provider closed the laundry room door and put a safety cover on the door knob during the visit.

It has been discussed that her sister would become a substitute but specialist hasn't received a substitute form or a notarized release in order to approve her. Provider gave the specialist the completed substitute form but her sister needs to complete a release form. Provider needs to submit the release form ASAP.

Findings: Provider had the tv on when specialist arrived at 7:53am and it didn't go off until the group went to the bus stop at 8:20am. Children were watching it. Provider is reminded that the tv should not be on for a passive watching purpose for more than 30 minutes a week. The tv remained off when provider returned to the home.

Findings: Provider has a 4 month old enrolled. She states he can roll over. She placed him on his tummy for sleeping. Provider is reminded that children under 12 months of age need to be placed on their backs for sleep and the children may adjust to their own sleeping position. Provider moved the child to his back during the visit.
