Correction Required: A health assessment is valid for 24 months following the date of signature, if the person does not contract a communicable disease or develop a medical problem.

Noncompliance Area: The file for Staff #1 contained an expired First-Aid certificate. Staff #1 acknowledged that she did not have a current certificate.

Correction Required: Competence in first-aid is demonstrated by the completion of training by a professional in the field of first-aid. First-aid training shall be renewed on or before expiration of certification or every 3 years, as applicable. One or more persons competent in first-aid techniques shall be at the facility when day care children are in care.

Noncompliance Area: The file for Staff #1 did not contain documentation of the completion of 12 hours of training in the past two years.

Correction Required: A staff person shall obtain an biennial minimum of 12 clock hours of child care training.A facility person's record shall include records of training required by the Department.

Noncompliance Area: Along the side of the front porch, adjacent to the steps traversed by children as they enter and exit the facility, an approximately 18" long portion of aluminum siding was observed to have a sharp edge that could cause laceration injuries. The noted area of concern was positioned to be accessible to children in care.

Correction Required: Floors, walls, ceilings and other surfaces, including the facility's outdoor play area, shall be kept clean, in good repair and free from visible hazards.

Noncompliance Area: The file for Chico, the family dog, who is present at the residence when children are in care, contained an expired rabies immunization certificate.

Correction Required: A veterinarian's certificate of current rabies immunization is required for a cat or dog at the facility. The certificate shall be on file when the cat or dog is present.

Noncompliance Area: The file for Child #1 contained an emergency contact form that did not include a parent's signature giving consent for emergency medical care to be provided.

Correction Required: Emergency contact information must include the written consent signed by a parent for emergency medical care. A child's record must contain signed parental consent for emergency medical care for the child. Written consent is required prior to admission.

Noncompliance Area: The file for Child #1 contained an emergency contact form that did not include addresses for persons to whom the child can be released.

Correction Required: Emergency contact information must include the name, address and telephone number of the individual designated by the parent to whom the child may be released.

Noncompliance Area: In the dining room between an electric fireplace/mantel and a shelving unit, in an area accessible to children in care, an extension cord, which was secured at both ends, was observed. Said cord presented the potential for looping, which created a strangulation hazard.

Correction Required: Conditions at the facility may not pose a threat to the health or safety of the children.

Noncompliance Area: The file for Child #1 contained an emergency contact form that did not include a parent's signature giving consent for minor first-aid procedures.

Correction Required: A child's record must contain signed parental consent for administration of minor first-aid procedures by facility staff. Written consent is required prior to admission.

Noncompliance Area: Copies of applicable regulations and regional day care office contact information were not posted in a conspicuous location at the facility.

Correction Required: The facility's current certificate of registration and a copy of the applicable regulations under which the facility is certified shall be posted in a conspicuous location used by parents, with instructions for contacting the appropriate regional day care office posted at the same location

Noncompliance Area: The facility's emergency plan was not posted in a conspicuous location.

Correction Required: The operator shall post the emergency plan in the facility at a conspicuous location.

Noncompliance Area: In an area accessible to children under the age of six, an unused, unprotected electrical outlet was observed on a surge protector in the living room

Correction Required: Protective receptacle covers shall be placed in electrical outlets accessible to children 5 years of age or younger.

Noncompliance Area: Several cigarette butts were observed near porch steps to the entry/exit of the residence, which is traversed by children in care.

Correction Required: Ashes and cigarette or cigar butts are prohibited in a child care space, a play space or a food preparation area.

Noncompliance Area: In the dining room, in the aquarium cabinet, two unused, unprotected outlets were observed in a surge protector in an area accessible to children under the age of six.

Correction Required: Protective receptacle covers shall be placed in electrical outlets accessible to children 5 years of age or younger.

Noncompliance Area: Two gallon-sized containers of C-Balance, a chemical intended for use in aquariums, were observed. Located in an area accessible to children, the manufacturer's label read, Keep out of Reach of Children. In the bathroom, on a behind-the-toilet shelf, a pump spray bottle of air freshener labeled Keep Out of Reach of Children was observed.

Correction Required: Cleaning materials and other toxic materials shall be stored in an original labeled container or in a container that specifies the content. Toxics shall be kept in a locked area or in an area inaccessible to children, and shall be stored away from food, food preparation areas and child care spaces.
