Noncompliance Area: The facility's COVID-19 health and safety plan lacked written screening procedures.

Correction Required: Conditions at the facility may not pose a threat to the health or safety of the children.

Noncompliance Area: There was more than one year between the previous and most current review of the emergency plan. Dates are follows: 9/25/29 and 10/9/20.

Correction Required: The operator shall review the emergency plan at least annually and update the plan as needed. The operator shall document in writing each review and update of the emergency plan.

Noncompliance Area: There was more than 60 months between the previous and most current Child Abuse clearances on file for household member #2. Dates are as follows: 5/22/15 and 6/2/20.

Correction Required: The operator shall comply with the CPSL and with Chapter 3490 (relating to protective services). A facility person's record shall include a copy of requests for the criminal history record and child abuse registry clearance information, a copy of the disclosure statement and a copy of the completed clearance information required under the CPSL.

Noncompliance Area: There was more than 60 months between the previous and most current mandated reporter training on file for staff #1. Dates are as follows; 3/1/15 and 8/15/20.

Correction Required: The operator shall comply with the CPSL and with Chapter 3490 (relating to protective services). A facility person's record shall include records of training required by the Department.

Noncompliance Area: The hands-free can used by the facility to store discarded diapers was observed to lack a plastic lining.

Correction Required: If disposable diapers are provided by a parent or by a facility, a soiled diaper shall be discarded by immediately placing the diaper into a plastic-lined, hands-free covered can.

Noncompliance Area: In the downstairs bathroom, atop the sink, in an area accessible to children in care, an aerosol can containing air freshener labeled Keep Out of Reach of Children was observed.

Correction Required: Cleaning materials and other toxic materials shall be stored in an original labeled container or in a container that specifies the content. Toxics shall be kept in a locked area or in an area inaccessible to children, and shall be stored away from food, food preparation areas and child care spaces.

Noncompliance Area: The first aid kit in the child care space contained non-sterilized gauze.

Correction Required: A first-aid kit must contain the following: soap, an assortment of adhesive bandages, sterile gauze pads, tweezers, tape, scissors and disposable, nonporous gloves.

Noncompliance Area: Two approximately 1.5" pointed screws and two lighters were observed in an unsecured side table drawer in the living room in an area that is accessible to children in care.

Correction Required: Conditions at the facility may not pose a threat to the health or safety of the children.

Noncompliance Area: A spray can of Scrubbing Bubbles cleanser labeled Keep Out of Reach of Children was observed in an unsecured bathroom cabinet that is accessible to children in care.

Correction Required: Cleaning materials and other toxic materials shall be stored in an original labeled container or in a container that specifies the content. Toxics shall be kept in a locked area or in an area inaccessible to children, and shall be stored away from food, food preparation areas and child care spaces.

Noncompliance Area: The file for Child #1 contained a current health assessment; however, the previous health assessment was completed eight months ago when the child was a young toddler requiring health assessments to be completed every six months.

Correction Required: The operator shall require the parent to provide an updated health report at least every 6 months for an infant or young toddler

Noncompliance Area: Three diaper cream containers in the downstairs child care space and a bottle of Children's Allergy Relief in the kitchen lacked a label with the child's name.

Correction Required: The label of a medication container shall identify the name of the medication and the name of the child for whom the medication is intended. Medication shall be administered to only the child whose name appears on the container.

Noncompliance Area: A bottle was observed in the refrigerator that lacked a label with the child's name.

Correction Required: Disposable nursers and bottles shall be labeled with the child`s name.

Noncompliance Area: In the entry hallway, two pieces of broken glass were observed near the base of the wall. Pieces were approximatley 8"x10" and 11"x17" and in an area accessible to children.

Correction Required: Conditions at the facility may not pose a threat to the health or safety of the children.

Noncompliance Area: Five electrical outlets were observed to be lacking protective receptacle covers in areas accessible to children under the age of six. Unprotected outlets were observed in living room, each of the two upstairs bedrooms, dining room, and upstairs bathroom.

Correction Required: Protective receptacle covers shall be placed in electrical outlets accessible to children 5 years of age or younger.

Noncompliance Area: Approximately 15-20 nails were observed protruding through the walls in the entryway to the facility with the pointed ends of the nails exposed in areas accesible to children in care.

Correction Required: Floors, walls, ceilings and other surfaces, including the facility`s outdoor play area, shall be kept clean, in good repair and free from visible hazards.
