I will get a letter of exemption for flu shots from parents of Child #1, #2 and #4. Child #3 has to give me an initial health assessment, which will include her immunization record. Child #2 did not go to a doctor for over a year and appears to be on a catch-up schedule. Parent will provide letter from doctor with an explanation and any updated immunizations that have taken place since his last health assessment.
