The following was discussed, but not limited to: 1) Tummy time rules, 2) Transitioning of infants is not allowed between the one-year-old room and the infant room, 3) The new rules that are effective 8/3/2025, and 4) The annual number of training hours for staff is now 24 hours. There were 4 staff files reviewed. 4 of 4 fingerprint clearance cards were verified to be valid through the DPS website.

Initial Comments: The following deficiencies were found at the time of the Compliance Inspection conducted on 8/16/2024, and are subject to changes pending programmatic review. Name of Compliance Officer: Jennifer Flicker The Written Documentation is required to be submitted through the Licensing Portal within 10 days of the receipt of the Statement of Deficiencies. The following was discussed, but not limited to: *Due to reasons of sanitation, items stored in a bathroom are required to be stored inaccessible to children in either a closed cabinet or in a waterproof container that is covered with a lid, *The Empower program no longer subsidizes the fee, and * Submitting the City Of Phoenix fire inspection when it is completed later this month. There were 5 staff files reviewed. 5 of the 5 fingerprint clearance cards were verified to be valid through the DPS website. The Empower Assessment was completed at the time of the inspection.

Initial Comments: The following deficiencies were found at the time of the Compliance Inspection conducted on 8/22/2023, and are subject to changes pending programmatic review. Name of Compliance Officer: Jennifer Flicker The Written Documentation is required to be submitted through the Licensing Portal within 10 days of the receipt of the Statement of Deficiencies. **Please submit the following documents: 1) Current City of Scottsdale Fire inspection, 2) The Certificate of Insurance when renewed next month, 3) Foreign degree verification, and 4) GED documentation when staff completes. The following rules were discussed: 1) Tummy time rule and the requirement, and 2) The menu details required to be listed for food items. There were 4 staff files reviewed. Four of the 4 fingerprint clearance cards were verified to be valid through the DPS website. The Empower Survey was completed at the time of the inspection.
