|Findings: After a review of 5 staff files, it was determined the following was lacking: Staff #1 (start date 3/9/2020) had documentation of freedom from infectious active tuberculosis. The TB test in Staff #1 file, did not have a result.|

|Findings: Findings include: In the Sprouts room the Surveyor observed two large containers of red and blue paint and a clipboard on top of the diaper changing counter. In the Berry Patch 1 room the Surveyor observed a water pitcher, a glass cup next to the hand washing sink for diapering. On the diaper counter was a piece of a wood.|
|Findings: Findings include: The Surveyor reviewed 7 staff files and found 4 files lacking the following: Staff #1 (start date 5/28/19), Staff #2 (start date of 6/4/19), Staff #3 (start date of 7/8/19) and Staff #4 (start date of 6/5/19) files lacked documentation for the required new hire 10 day staff training.|
|Findings: Findings include: Per allegation and based on documentation, staff interviews and the Surveyor and the assistant director viewing the video: Staff #2 requested that Child #1 to go to the restroom. When the child refused by laying on the floor, the staff member picked the child up around the waist and from behind and carried the child to the restroom. The staff member waited at the door of the restroom while the child stood in the restroom crying. The staff member then entered the restroom and pulls down the child's clothing and places the child on the toilet. Staff #2 stated that the class was getting ready to go outside and the staff member wanted the children to use the potty before going outside. Staff #1 stated the staff member received one on one training with the assistant director on 8/30/18 and 9/10/18. Training involved proper lifting techniques, appropriate expectations and interactions with children. The staff member watched videos to incorporate in classroom management and conscious discipline. Will encourage staff member to seek additional training.|
