| Findings: Findings include: Per allegation and based on documentation, staff interviews and the Surveyor and the assistant director viewing the video: Staff #2 requested that Child #1 to go to the restroom. When the child refused by laying on the floor, the staff member picked the child up around the waist and from behind and carried the child to the restroom. The staff member waited at the door of the restroom while the child stood in the restroom crying. The staff member then entered the restroom and pulls down the child's clothing and places the child on the toilet. Staff #2 stated that the class was getting ready to go outside and the staff member wanted the children to use the potty before going outside. Staff #1 stated the staff member received one on one training with the assistant director on 8/30/18 and 9/10/18. Training involved proper lifting techniques, appropriate expectations and interactions with children. The staff member watched videos to incorporate in classroom management and conscious discipline. Will encourage staff member to seek additional training. | |||
