| Operator did not provide a physically safe and healthy indoor and outdoor environment that meets the developmental needs of the children in care. A small hole was observed just above the baseboard in the hallway. On the playground, the top fence rails and greater than two fence posts bordering the outdoor play environment was observed to be heavily rusted. One area of the fencing had become detached from the poles and was observed to be unstable. A rusted chain was observed hanging from the back left corner of the fence (facing away from the home). A wooden plank with thin, metal threads, sharp to the touch was observed lying on top of a piece of rusted fencing toward the back right of the outdoor play area (facing away from the building). Goat fecal pellets were observed to be emerging from chicken wire that lined the bottom of the structure, accessible to children. Areas of rust were observed to be forming on nuts and bolts of one nonstationary play structure and one jungle gym, as well as on a swing set. On the swing set, a small bar used for holding and swinging was observed to have a rusted hold at the base which was sharp to the touch. Two outdoor chairs were observed to be rusting. A sit-and-bounce ball with handle was observed to be covered with mildew. A rusted shovel was observed leaning against the inside of the fence toward the back of the play area; Ms. Mabe placed this shovel on the other side of the fence during the visit.
