|Findings: Provider did not submit the required Release of Information form for returning 18 year old resident grandson as required. Grandson had moved out in July 2018 and office was notified. Provider states that he moved back into the home around December 2018. Provider submitted copy of Release of Information at time of inspection for correction.|
|Findings: Provider had not reviewed and updated the home's written emergency and disaster plan at least annually as per regulations. Provider reviewed and initialed and dated for correction at time of inspection.|
|Findings: Specialist observed 2 uncapped outlets in the approved child care living room area at time of inspection. Provider corrected at time of inspection and will monitor on an ongoing basis to ensure safety and compliance.|
|Findings: Provider did not have returning 18 year old grandson fingerprinted as required within 5 working days of his return to the child care home in December 2018. Provider will ensure that grandson submits FBI and MD fingerprints using Office of Child Care Region 3 authorization number 1100000031. Provider stated she will take her grandson this week and will notify office upon completion.|
|Findings: Specialist observed The Works brand toilet bowl cleaner in a unlocked cabinet in the bathroom under the sink at time of inspection. Provider stated the lock had been on and found the child proof lock and corrected at time of inspection. Provider will monitor on an ongoing basis to ensure safety and compliance.|
|Findings: Provider's monthly file drill documentation was observed at time of inspection. Provider stated that she completed an emergency drill this past summer but did not document. Provider will complete a minimum of 2 emergency drills per year and document as per regulations. Provider will submit copy of documentation of emergency drill for correction by March 31, 2019|
|Findings: Provider must have parent update address on emergency cards of her 4 children in care as two different addresses are listed between the four children and provider states that she has moved to another address. Provider will remind parents to update emergency cards as applicable. Parent will then date and initial as indicated on form. Provider will submit copies of all 7 emergency cards for correction prior to March 31, 2019.|
|Findings: Provider did not submit the required medical for returning resident grandson as required within 15 working days. Provider will submit documentation of a medical evaluation appointment prior to March 26, 2019 and will submit original medical to office for correction upon completion.|
|Findings: Provider had not completed her 12 hours of training for her 2017-2018 anniversary year. As provider completed her 2016-2017 training for correction on 6/16/2017 she was to have completed 12 hours from 6/17/2017 to 3/31/2018. Provider has only completed 2 of those hours. Provider will correct by completing a 10+ 12 additional hours by March 31, 2019 and submit copies of all certificates documented on and attached to the Professional Development Plan.|
|Findings: Specialist observed some exposed tacking at the edge of the floor at the doorway going downstairs to the main child care area. Carpeting surrounded the area except for the exposed wood and tacking in a small area and provider stated she was going to replace with a carpet piece. Provider will correct immediately and will submit photo for correction within 5 working days.|
