Findings: Specialist observed fallen tree branches as well as wood boards and loose lattice stacked against the fence in the child care yard. Specialist observed two exposed nails on the wooden boards against the fence. Specialist also observed a pile of cords at the base of a tall tree in the center of the child care yard and also loose plug hanging where a light fixture is mounted higher on the tree. The gate at the top of the child care entrance stairs which also leads from the outdoor play yard to the child care entrance was observed to be in disrepair and did not close due to disrepair. A rusted nail was observed on the board where the gate is mounted on one side and a rusted screw was observed protruding from the board where the gate is mounted on the other side-both within reach of children. A broken walker was observed in the outdoor play yard; walkers should not be used in child care and any materials or equipment not in good repair should be disposed of. Provider will correct all items immediately and will not use yard until it is in safe condition. Provider will submit a letter of correction to office within 5 working days. Provider will monitor on an ongoing basis for safety.
