Findings: Specialist did not observe physician information on 4 of the 8 emergency cards of children in care. Provider will have parents complete at pick up and will submit copies of emergency cards to office for correction, via scan and email or mail, within 5 working days for correction.

Findings: Specialist did not observe documentation of immunizations for one child in care (although provider stated that she is certain she received, but could not locate at time of inspection), physician portion of health inventory for another child in care and lead screening for two children in care. Of the two children in care for whom the specialist did not observe lead screening, both began prior to their 1st birthday. As per regulations, they should have a lead screening at their 1st and 2nd birthdays. Providers are required to track and request lead screenings of those children who begin prior to their 1st birthday. Specialist also did not observe documentation of individual care needs for four children in care. Provider will correct immediately and submit correction copies of forms to office for correction within 10 working days.
