Findings: Specialist observed an Epi-Pen that expired 10/2019 and Albuterol that had expired 3/2019 for a child in care. Provider will return to parent immediately and get updated medication as applicable. Provider will submit a letter of correction to office as soon as possible. If child still is in need of medication, the child may not return to care without medication and current medication order form as outlined in the regulations.

Findings: Specialist observed the outdoor play structure to be in need of some repair. The bottom step of the ladder up to the equipment is broken. A screw on the equipment was observed to be protruding and some areas were observed to have peeling paint. Provider will inspect all areas of the equipment and correct prior to children using again. Provider will submit photos of correction to office within 10 working days.

Findings: Specialist did not observe the Additional Adult wash her hands after diapering a child in care. Additional Adult had gloves on and removed the gloves, but hand washing is still required as per regulations. Additionally, a child's hands must be washed thoroughly by the provider with soap and warm running water after diapering and other times as outlined in regulations. Wiping a child's hands with a wipe is only approved for a small infant who does not have head control yet.

Findings: Specialist observed the soap in the basement child care bathroom to be on a ledge above the sink and not within reach of a child capable of using the toilet without assistance. Provider stated that she squirts the soap in the child's hands, but as per regulations it must be within reach. Provider will correct immediately and submit a statement of correction to office within 5 working days. Provider would like to discuss this non compliance with supervisor.

Findings: Once child in care did not have documentation of a lead screening as required by his birthdate of 6/22/2015. Another child had documentation of her 1st but not second lead screening. One child in care had a medication order form that was not updated by doctor at least every 12 months as required. This medication order form was signed 3/19/2018 and must be updated at least every 12 months or sooner if applicable. Provider will submit as soon as possible for correction. Child may not return to care without current prescriptions and medication order form as outlined in the regulations.
