School-aged children were not adequately supervised. During today's visit, I observed a group leader escort sixteen (16) children to the restroom located in the hallway and then transitioned to the outdoor play area. While the group was outdoors, I observed a five year old child walk into the cafeteria from the restroom unaccompanied by an adult. The group leader was not aware of the child's whereabouts since the child was not with the group when they left the restroom and went outdoors. | |||
