A safe indoor and outdoor environment was not provided for the children. In the outdoor play area used for preschool aged children, the gray platform deck had boards that were not flush, causing a trip hazard. Cement footings used to anchor the shade structures were exposed and caused a trip hazard. A black substance was observed on each shade structure, metal posts, and toy balls used by the children. On the playground used for school-aged children, the concrete footings securing a basketball goal were exposed and broken, causing the structure to be unsecure. The concrete platform with base surrounding it exposed sharp edges and a six (6) inch hole due sand erosion. |
