Findings: Health and Safety. There was a concern of a staff member taking inappropriate measures to deescalate a child with behavioral issues. An investigation found that a child's behavior was endangering others and a staff member laid next to a child but not on the child. Resolution: The facility has individual behavior plans with families and staff are all aware of plan. The provider is implementing a new plan with a behavior specialist working together with classroom staff and will review each child once a month at staffing meetings.
