Description: The Kid Zone Enrichment Program is a dynamic community response to changing lifestyles and needs of families in Tempe, Arizona. Our goal is to address the increased number of children needing a safe, enriching place to be in out-of-school time. Kid Zone operates both before and after school programs at 19 Tempe and Kyrene Schools. Our program is for children in the grades K-5 at 15 of our schools and K-8 at 3 of our schools (Curry, Rover, Ward). We also offer a Pre-School Program for 2 year olds - Pre-K's.

Findings: Based on a review of the files for the 2 staff listed as the Director's designees on the parent posting board (staff #2 and staff #3), neither staff were at least east 21 years old and therefore neither staff meet the minimum qualifications required as a director designee. At the time of the inspection, staff #2 stated that the Director was on vacation and staff #2 was acting as the Director's designee.

The Staff stated he thought the urinal was either leaking or overflowing since the floor had been wet the prior day too.

Findings: Based on the Surveyor's observation, the sand measured the following depths: *Less than 1 of the required 6 inches at the bases of the 2 red slides; *Less than 2 of the required 6 inches under the yellow climbing structure and under the faux rock climbing wall; * Less than 2 of the required 6 inches under the yellow climbing stairs; and *Less than 4 of the required 6 inches under the yellow monkey bars.

Findings: Based on Surveyor's observation and a review of staff #2's file, it was determined that the file lacked documentation of the staff's current fingerprint clearance card. Per the Department of Public Safety website, staff #1 has a valid fingerprint that is valid until 8/5/2022.

Findings: Based on a review of 2 staff files, the Surveyor determined the files for staff #1 and #2 lacked documentation of the staff's Central Registry Background Clearance.

Findings: Based on the Surveyor's review of the licensee's City of Tempe fire inspection that was completed on 10/25/2019, the Surveyor determined the fire inspector had cited 5 violations and the fire inspection was not approved.
