Findings: Findings include: Based on the Surveyor's observations, review of the facility documentation, and information provided by the staff at the time of the complaint investigation, the following rosters did not reflect the number of enrolled children present in the activity area: *In Room 6, there were 5 enrolled children present in the room and only 6 enrolled children were documented on the roster. *In Room 16, there were 4 enrolled children present in the room and only 3 enrolled children were documented on the roster. *In Room 19, there were 8 enrolled children present in the room and only 7 enrolled children were documented on the roster. Note: On 12/15/2021, Staff #1 failed to account the temporary absence of Child #1 while in the room and the outdoor play area. Based on staff interviews, Child #1 left the room without Staff #1's knowledge and was brought back to the room by the Occupation Therapist before they went out to the outdoor activity area. While the class was in the outdoor activity area, Child #1 left the facility premises without Staff #1's knowledge and was found twenty minutes later by a staff member.

Findings: Findings include: Based on the Surveyor's observations, review of the 12/15/2021 campus video footage, and 4 staff interviews, it was determined on 12/15/2021, Staff #1 did not maintain the required ratio of staff to 3-year-old children while on the outdoor activity area between 9:45 am-10:45 am. The ratio was 0:1 when Child #1 left the facility premises and was found .02 miles away at a home by a staff member. Staff #1 stated that they waited for Staff #2 to come back from taking an enrolled child to the restroom before they notify the principal that Child #1 was missing. Staff #1 stated it took 10 minutes before they let the principal know of the situation. Twenty five minutes later, Child #1 was found .02 miles away at a home by a staff member. Based on staff interviews: Staff stated that Child #1 had previously left the room earlier in that day while they stepped out of the room to take another enrolled child to the restroom. When they got back into the room, they noticed that Child #1 had left the room and the Occupation Therapist knocked on the room's door to bring Child #1 back to the room. Staff stated that Child #1 has left the room various times in the school year.

Findings: Per allegation Findings include: Based on the Surveyor's observations, review of the 12/15/2021 campus video footage, and 4 staff interviews, it was determined that the licensee did not ensure the health, safety, and welfare of Child #1 who was placed at risk of harm. On 12/15/2021, while the enrolled children in room 6 played in the outdoor activity area, Staff #2 left the outdoor activity area for 10-12 minutes to take an enrolled child to the restroom leaving Staff #1 alone with 11 enrolled children in the outdoor activity area. During that 10-12 minute timeframe, Child #1 managed to leave the outdoor activity area without Staff #1's knowledge. The campus video footage showed Child #1 wondering in the staff parking lot behind the school for over one minute without any staff supervision. Staff #1 waited for Staff #2 to come back to the outdoor activity area to notify the principal that Child #1 was missing. Twenty five minutes later, Child #1 was found .02 miles away at a home by a staff member. Based on the staff interviews, Staff #1 stated "they did not know how Child #1 escaped the outdoor activity area. Child #1 may have climbed the fence or reached the push bar door to leave".

Findings: Based on Surveyor's observation and on a discussion with the Preschool Program Manager, none of the 11 approved diaper changing areas were equipped with the bleach water bottles that is required to sanitize the diapering areas. The diapering areas were still only equipped with bottles of a disinfecting solution named "Re Juv Nal". This product is not a sanitizing agent, nor is it approved by the Department. *Note: This deficiency was also cited at the time of the previous compliance inspection that was conducted in 11/2020.

Findings: Based on a review of staff 1's file (date of employment 7/1/19), the Surveyor determined that the file contained documentation of the staff's freedom from infectious active tuberculosis that was completed on 7/18/2019. The staff file lacked documentation of the staff's freedom from infectious active tuberculosis on or before the staff's starting date of employment.
