Noncompliance Area: On July 17, 2026 an unannounced inspection was conducted at the facility, staff person #1 was observed in a direct childcare position and their health assessment did not contain the results of an examination for communicable diseases and the health care providers assessment of their suitability to provide childcare.

Correction Required: An adult health assessment must include an examination for communicable diseases and the results of that examination. An adult health assessment must include the physician's or CRNP's assessment of the person's suitability to provide child care.

Noncompliance Area: On July 17, 2026 an unannounced inspection was conducted at the facility, at the time of the inspection, Staff person # 1 was observed to be caring for children unsupervised. Staff person #1 has not completed the following pre- service training required prior to caring for children unsupervised: mandated reporter training.

Correction Required: Conditions at the facility may not pose a threat to the health or safety of the children. Until such time as the required pre- service trainings are completed, staff person #1 must be supervised, when interacting with children at a minimum by, a family child care home staff person who has completed all preservice trainings and has all qualifications to care for children unsupervised. If there are no staff person(s) available to supervise staff person #1, staff person #1 may not work in a child-care position at the facility.

Description: Locked or inaccessible - in original container

Noncompliance Area: On July 17, 2026 an unannounced inspection was conducted at the facility, a spray can of air freshener was found on a book case that was within the reach of the children.

Correction Required: Cleaning materials and other toxic materials shall be stored in an original labeled container or in a container that specifies the content. Toxics shall be kept in a locked area or in an area inaccessible to children, and shall be stored away from food, food preparation areas and child care spaces.

Noncompliance Area: A renewal inspection was conducted at the facility on March 10, 2026. At the time of the inspection a review of children's files was conducted, and Child#1 was missing information on who shall be contacted in an emergency.

Correction Required: Emergency contact information is required for each enrolled child. Emergency contact information must reference who shall be contacted in an emergency.

Correction Required: Emergency contact information must include the name, address and telephone number of the child's physician or source of medical care.

Noncompliance Area: A renewal inspection was conducted at the facility on March 10, 2026. At the time of the inspection a review of children's files was conducted, and Child #2 was in care at the time of the inspection, and their file was last updated in 2024 from a previous location. Child #3 was also in care at the time of the inspection and did not have a file at the facility.

Correction Required: An operator shall establish and maintain an individual record for each child enrolled in the facility. Information in a child's record shall be kept current by the operator.

Noncompliance Area: An initial inspection was conducted at the facility of 11/21/25, at that time outlets in the front room were observed to not have protective covers.

Correction Required: Protective receptacle covers shall be placed in electrical outlets accessible to children 5 years of age or younger.

Noncompliance Area: An initial inspection was conducted at the facility of 11/21/25, at that time the kitchen stove was accessible to children.

Correction Required: Hot water pipes and other sources of heat exceeding 110° F that are accessible to children shall be equipped with protective guards or shall be insulated to prevent direct contact.

Regulation: 3290.70(d)

Description: Above ground window opening restricted to 6"

Noncompliance Area: An initial inspection was conducted at the facility of 11/21/25, at that time the second-floor bathroom window was able to be opened more than 6 inches.

Correction Required: Windows or doors above the ground floor that open directly to the outdoors and are accessible to children shall be constructed, modified or adapted to limit the opening to 6 or fewer inches.

Description: Requirements

Noncompliance Area: An initial inspection was conducted at the facility of 11/21/25, at that time a piece of glass was broken out of the front door where children will enter and exit the facility.

Correction Required: Floors, walls, ceilings and other surfaces, including the facility's outdoor play area, shall be kept clean, in good repair and free from visible hazards.
