First aid kit and/or fire extinguisher was not located in vehicle; kit and/or extinguisher was not mounted or secured if in passenger area. A first aid kit was not located in the van. Fire extinguisher was not secure, it was located in a plastic bin sitting beside the driver's seat in the middle front of the van.
