A safe indoor and outdoor environment was not provided for the children. On playground space #3, there was a blue outdoor equipment cart which was not sturdy and is accessible to the children. The metal bars on the second tier were rusty and detached. On playground space #1, there were protruding screws observed on the steps used to climb up to the slide structure. The wood structure attached to the steering wheel was wobbly and not secure.

A bleach cleaner was observed in an unlocked cabinet in space #7.

A safe indoor and outdoor environment was not provided for the children. Three pills, red and blue in color, were found by a 4 year old child on the playground used by the preschoolers.

Equipment and furnishings were not sturdy, stable and free of hazards. Outdoors, there were several concerns observed. There was a rusty muffin pan hanging on the wood panel in the music center, a rusty pole hanging on the side of the building, and rusty hooks in the kitchen area. There were protruding screws near the steering wheel and on the slide platform.
