| The indoor and/or outdoor premises was not checked once a day, prior to initial use, ensuring debris, and broken equipment was removed and disposed of. On the playground used by the infant children, the overhang has nails protruding with a nest in the corner by the door of the infant classroom. The tension band has loosened resulting in the lower half of the fence starting to curl that has exposed sharp rusting edges that are accessible to the children. On the playground for the one-year-old children, broken plastic sand spinning toy and walking poppers were accessible to children. On the playground for the 2 and 3 year old children, the wooden bench was broken and accessible to children. On the playground for the preschool age and after school age children, their was trash on the ground, a broken shelf used to store toys, a broken green plastic planter accessible to children, and there was a hole between the fence and the concrete. On all the playgrounds, the 4X4 post have exposed rough edges and have started to splinter. | |||
