Noncompliance Area: Aquafor, A&D Ointment, Butt Paste and diaper rash cream, all labeled "keep out of the reach of children" were on the floor accessible to children in care.

Correction Required: Cleaning materials and other toxic materials shall be stored in an original labeled container or in a container that specifies the content. Toxics shall be kept in a locked area or in an area inaccessible to children, and shall be stored away from food, food preparation areas and child care spaces.

Noncompliance Area: The files of child #1, #2, #3 and #5 contained documentation of current forms providing the families with information regarding the child's growth and development but did not contain documentation of the prior assessment to determine if they were completed within the last 6 months. Based on a conversation with the provider, the previous assessments were given to the parent when the new was completed.

Correction Required: An agreement shall specify the services to be provided to the family and the child, including the Department's approved form to provide information to the family about the child's growth and development in the context of the services being provided. the operator shall complete and update the form and provide a copy to the family in accordance with the updates regarding emergency contact information at § 3290.124(e).

Noncompliance Area: The file of child #4 contained an immunization record with did not contain documentation of 1 rotavirus, 1 DTAP, 1 Hib, 1 pneumococcal, and 1 polio vaccination as recommended by the ACIP or an acceptable exemption.

Correction Required: A health report shall include a review of the child's immunized status according to recommendations of the ACIP. The facility shall implement dismissal policies in accordance with the Department of Health regulation in 28 Pa. Code § 27.77 (relating to immunization requirements for children in child care group settings).

Noncompliance Area: The file of child #1, #2, #3 and #5 contained documentation of emergency contact forms and financial agreements being reviewed and/or updated within the last 6 months but did not contain documentation of the previous form and agreement to determine if it was completed within the 6 months of those. Based on a conversation with the operator, the previous emergency contact form and financial agreement were returned to the families once the current ones were obtained.

Correction Required: A parent is required to review and update the emergency contact information and the financial agreement at least once in a 6-month period or as soon as there is a change in the information.
