information.) I requested from Child #1's parent a health assessment that meets the state of Pennsylvania's criteria. Child #1 just moved back from Colorado to the area and did have a health assessment that did state she was cleared to attend Daycare/Schools & Summer Camps. The assessment lacked appears to be free from contagious or communicable disease. The parent provided me a copy of a new form filled out on 10/29/2020 stating that she is able to participate in childcare and appears to be free from contagious or communicable disease and at the bottom of the form the parent put in writing that Child #1 will not receive the flu shot due to our family beliefs. She signed it and printed her name. Moving forward I will make sure that any child entering child care from another state has the required form from the state of Pennsylvania filled out before attending child care so that this does not happen again. I did send via email on November 3, 2020 at 2:21 P.M. a copy of Child #1's health assessment with the note on the bottom of the form highlighted. I also included in the email all my plans of correction. |

I spoke to the parent in regards to the 3 year old not having verification of the influenza vaccination at pick up time on the date of inspection on October 22, 2020.The parent informed me that due to family beliefs that her child would not be receiving this shot. She put that in writing on the corrected Health assessment form that I did send over via email on November 3, 2020 at 2:21 P.M. it was highlighted on the bottom of the form and states Child #1 will not receive the flu shot due to our family beliefs. The parent signed and printed their name. Moving forward I will make sure that I address the influenza vaccination and make sure that the proper documentation is provided if a child is not receiving a vaccination. |

information.) I had a physical scheduled prior to 11-24-2019 and unfortunately my appointment got rescheduled by my physician's office. I took the first appointment that they could give me, which was 12-2-2019. Moving forward I will make sure that my health assessment is done in accordance to 55 PA Code Chapter 3290.151(a)(2) and that I give myself at least a two month window so this does not happen again. |

Do not take your child here if they are experiencing stranger awareness - the provider is not not equipped to handle your child in this state. She quit on the first day and was in tears and shaking upon pickup. She told me there was "something wrong with my son" who has historically gotten comments on his jovial demeanor and has reached all of his milestones. She also mentioned that she was unable to take care of the other children due to my son's crying, which is extremely concerning. If my son was in pain, which she claims he may have been, she was unable to soothe him, and I was not contacted to come pick him up.
