Noncompliance Area: In the front outdoor play space, observed a play farm house which had a 4 inch cracked area on the right front of the house. The crack has rough edges and sharp corners.

Correction Required: Toys, play equipment and other indoor and outdoor equipment used by the children shall be clean, in good repair and free from rough edges, sharp corners, pinch and crush points, splinters and exposed bolts.

Noncompliance Area: In the preschool room, observed a five foot white storage cabinet which was not secured to the wall and could be pulled over on the children.

Correction Required: Furniture must be durable, safe, easily cleaned and appropriate for the child's size, age and special needs.

Noncompliance Area: The emergency contact information on file for child #1 lacks the address of all individuals designated by the parent to whom the child may be released.

Correction Required: Emergency contact information must include the name, address and telephone number of the individual designated by the parent to whom the child may be released.

Noncompliance Area: There was more than 24 months between the previous and most current and updated health assessment on file for staff #1.

Correction Required: A facility person providing direct care who comes into contact with the children or who works with food preparation shall have a health assessment conducted within 12 months prior to providing initial service in a child care setting and every 24 months thereafter. A health assessment is valid for 24 months following the date of signature, if the person does not contract a communicable disease or develop a medical problem.A facility person's record shall include a written report of initial and subsequent health assessments, including the results of initial and subsequent tuberculin skin tests, x-rays or other medical documentation necessary to confirm freedom from communicable tuberculosis.

Noncompliance Area: The facility's van, which is used to transport the children, lacks manufacturer's instructions for the use of safety restraints.

Correction Required: Manufacturers' instructions for use of safety restraints shall be kept in the vehicle at all times.

Noncompliance Area: The file for staff #4 lacks two written, non family references.

Correction Required: A facility person's record shall include two written, nonfamily references from individuals attesting to the person's suitability to serve as a facility person

Noncompliance Area: On 11/5/2020, it was observed that there was not an operable fire detection system because a smoke detector in the Toddler room and a smoke detector in the Infant room did not sound when activated.. This condition at the facility poses a threat to the health or safety of children.

Correction Required: Conditions at the facility may not pose a threat to the health or safety of the children. Demonstration of Compliance with 34 Pa. Code 403.23 is required. The provider will demonstrate or document an operable fire detection system to an agent of the department (a more specific directed plan can be added if needed).

Noncompliance Area: The files for staff #2 and 3 lacked mandated reporter training within 90 days of hire. Staff #2's hire date is 3/12/20 and mandated reporter training is dated 10/30/20. Staff #3's hire date is 1/6/20 and mandated reporter training is dated 6/4/10.

Correction Required: The operator shall comply with the CPSL and with Chapter 3490 (relating to protective services). A facility person's record shall include verification of child care experience, education and training following the outset of service at the facility.

Noncompliance Area: The files for staff #1 and 2 lacked an NSOR by 7/1/20. There is now an NSOR on file and dated as such: Staff #1 - 7/17/20 and Staff #2 - 7/17/20. The file for staff #3, with start date of 1/6/20, lacked an NSOR and State Police clearance on or before initial start date in a child care position. There are now clearances on file and dated as such: NSOR - 3/9/20 and State police - 6/2/20. The file for household member #5 lacked an NSOR by 7/1/20 and lacks a State Police, Child Abuse, and FBI clearance. There is an NSOR on file and dated 7/17/20.

A facility person's record shall include a copy of requests for the criminal history record and child abuse registry clearance information, a copy of the disclosure statement and a copy of the completed clearance information required under the CPSL

Noncompliance Area: The file for staff #3, who holds the title of assistant group supervisor, lacks a high school diploma. There are 2500 hours of child care experience on file.

Correction Required: An assistant group supervisor shall have a high school diploma or a general educational development certificate and 2 years experience with children.A facility person's record shall include verification of child care experience, education and training prior to service at the facility.

Noncompliance Area: A plastic basket containing play figurines was cracked and broken exposing children to sharp and rough edges.

Correction Required: Medication shall be stored in accordance with the manufacturer's or health professional's instructions on the original label.

Noncompliance Area: The record of facility person #1, with a starting date of 7/16/2019, contained a child a completed child abuse clearance dated 7/17/2019 but did not contain a copy of the request. Facility person #1 now has current clearances.

Noncompliance Area: The file of child #1 contained a health report dated 5/7/2018 which did not contain a review of the child's immunization report or a statement the child is able to participate in child care and appears to be free from contagious or communicable disease. A completed health report dated 10/16/2018 is now on file for child #1.
