A facility person's record shall include a copy of requests for the criminal history record and child abuse registry clearance information, a copy of the disclosure statement and a copy of the completed clearance information required under the CPSL. Description: Requirements

Noncompliance Area: On the outdoor play space on the right side, there was a green metal lattice that had sharp edges in multiple areas near the wooden fence and there was a vehicle tow attachment on the ground at the end of the driveway that had sharp edges in multiple areas that were accessible to children.

Correction Required: Floors, walls, ceilings and other surfaces, including the facility's outdoor play area, shall be kept clean, in good repair and free from visible hazards.

Noncompliance Area: On the outdoor play space, there were multiple wooden posts along the fence in the front yard that had multiple areas of peeling paint on the posts.

Correction Required: Peeled or damaged paint or damaged plaster is not permitted on indoor or outdoor surfaces in the child care facility.

Noncompliance Area: According to the log, the operator did not manually test the interconnected fire detection system in the facility at least once every 30 days between 12-19-25 to 1-23-26.

Correction Required: Fire detection devices or systems must be in compliance with standards established under section 1016(c) of the act (62 P.S. § 1016(c)). The operator or designated staff person who is responsible for compliance with this chapter shall ensure the requirements in subsection (a) are met.

Noncompliance Area: A preschool child was observed on a cot without 2 feet of space on three sides.

Correction Required: At least 2 feet of space is required on three sides of a bed, cot, crib or other rest equipment while the equipment is in use.

Description: Infant sleep position

Noncompliance Area: At approximately 9:20 a.m., child #1, an infant, was observed sleeping in a car seat in a stroller and was not placed in the sleeping position recommended by the American Academy of Pediatrics. The crib mattress for child #1 was observed to be inclined. Staff stated that the crib mattress was inclined due to a medical issue, and there was not a medical reason on file with a signed statement by a physician, physician's assistant or CRNP documenting the reason the child should not sleep in the sleeping position recommended by the American Academy of Pediatrics.

Correction Required: Infants shall be placed in the sleeping position recommended by the American Academy of Pediatrics unless there is a medical reason an infant should not sleep in this position. The medical reason shall be documented in a statement signed by a physician, physician's assistant or CRNP and placed in the child's record at the facility.

Noncompliance Area: In an unlocked drawer in the kitchen, there was a can of WD-40 labeled "Keep out of reach of children" and was accessible to children.

Correction Required: Cleaning materials and other toxic materials shall be stored in an original labeled container or in a container that specifies the content. Toxics shall be kept in a locked area or in an area inaccessible to children, and shall be stored away from food, food preparation areas and child care spaces.

Noncompliance Area: Staff person #1 was observed to be caring for children unsupervised. Staff person #1 does not have current pediatric first-aid/CPR training. This is evidenced by the previously documented pediatric first-aid/CPR training expiration date of October 2023.

Correction Required: Conditions at the facility may not pose a threat to the health or safety of the children. Until such time as the required trainings are completed, staff person #1 must be supervised, when interacting with children, by a family child care home staff person who has completed the required training related to this citation. If there are no staff person(s) available to supervise staff person #1, staff person #1 may not work in a child care position at the facility.

Noncompliance Area: The file for child #1 lacked a current agreement updated by the parent within the last 6 months.

Correction Required: A parent is required to review and update the emergency contact information and the financial agreement at least once in a 6-month period or as soon as there is a change in the information.
