Description: Building Surfaces

Noncompliance Area: On 7-17-2024 during the Renewal inspection, the wall near the children's tables on the main floor was dirty. On 8-12-2024 during an unannounced visit, the basement area was not set up for childcare. The operator acknowledged that there was a flood in the basement. A box containing a Christmas tree was near the stairs, as well as other stacked boxes. Rugs were folded on the floor and stacked on each other. Tables and chairs were stacked and leaning against the wall, creating an unsafe area for the children.

Correction Required: Floors, walls, ceilings and other surfaces, including the facility's outdoor play space surfaces shall be kept clean, in good repair and free from visible hazards.

Noncompliance Area: On 7-17-2024 during the Renewal inspection, the paint at the top of the stairs leading to the basement was chipping.

Noncompliance Area: At the time of the renewal inspection the certification representative observed that child #3 and 4 did not have an influenza immunization in 2022 or 2023. No exemption letter was observed for child #3 and 4.

Correction Required: The facility shall require the parent to provide updated written verification from a physician, physician's assistant, CRNP, the Department of Health or a local health department of ongoing vaccines administered to an infant, toddler or preschool child in accordance with the schedule recommended by the ACIP. Exemption from immunization for religious belief or strong personal objection equated to a religious belief shall be documented by a written, signed and dated statement from the child's parent or guardian. The statement shall be kept in the child's record.

A health assessment is valid for 24 months following the date of signature, if the person does not contract a communicable disease or develop a medical problem.
