Noncompliance Area: On 2-12-2025 during a Renewal inspection, the parent handbook did not include clothing and supervision policies.

Correction Required: The operator shall review with the parent, at the time of application, the facility's general daily schedule, hours for which care is provided, fees, responsibilities for meals, clothing, health policies, supervision policies, night care policies, dismissal policies, transportation and pick-up arrangements.

Noncompliance Area: On 10-11-2024 during an unannounced visit, it was verified by the provider that staff person #1 was left alone with children in care and did not have first-aid/CPR training on file.

Correction Required: Competence is the completion of training by a professional in the field of first-aid and CPR. All staff persons shall renew their certification in pediatric first aid and pediatric cardiopulmonary resuscitation (CPR) on or before the expiration of the most current certification. Noncompliance Area: On 10-11-2024 during an unannounced visit, staff person #1 did not have an FBI clearance on file.

Correction Required: The operator shall comply with the CPSL and with Chapter 3490 (relating to protective services). SAFETY PLAN: The operator is required to submit to the Children and Youth Agency a plan of supervision or alternate arrangement as required by the Child Protective Services Law. Noncompliance Area: On 2-12-2024 during the Renewal inspection, the facility did not have interconnected fire detectors.

Correction Required: Fire detection devices or systems must be in compliance with standards established under section 1016(c) of the act (62 P.S. § 1016(c)).

Noncompliance Area: At the time of the renewal inspection the certification representative observed that child #1, 3, and 5 did not have a health report completed by the physician.

Correction Required: A health report must be written and signed by a physician, physician's assistant or a CRNP. The signature must include the individual's professional title.

Description: Annual Immunization reporting to DOH

Noncompliance Area: At the time of the renewal inspection the certification representative observed that child #1-5 were missing a Flu shot in the last year.

Correction Required: The facility shall comply with the annual immunization reporting requirements in accordance with the Department of Health regulation in 28 Pa. Code §27.77.

Description: Building Codes/General Health and Safety

Noncompliance Area: At the time of the renewal inspection the certification representative observed that the facility did not have a written policy for, Shaken Baby and Abusive Head Trauma. The facility also had an emergency plan that did not identify the duties of the staff during an emergency or plans for continuity of operations.

Correction Required: The legal entity shall maintain ongoing compliance with the applicable requirements prescribed by the Department of Labor and Industry in 34 Pa. Code § 403.23 and under section 1016(c) of the act at all times following issuance of any certificate of compliance granted by the Department under this chapter. Conditions at the facility may not pose a threat to the health or safety of the children.

Noncompliance Area: At the time of the renewal inspection the certification representative observed that child #2 did not have signed parental consent for emergency medical care.

Correction Required: A child's record must contain signed parental consent for emergency medical care for the child. Written consent is required prior to admission.

Noncompliance Area: At the time of the renewal inspection the certification representative observed that child #1 did not have signed parental consent for administration of minor first-aid.

Correction Required: A child's record must contain signed parental consent for administration of minor first-aid procedures by facility staff. Written consent is required prior to admission.

Noncompliance Area: At the time of the renewal inspection the certification representative observed that the operator had an emergency plan that did not provides for accommodations of infants and toddlers, children with disabilities, and children with chronic medical conditions.

Correction Required: The facility shall have an emergency plan that provides for accommodations of infants and toddlers, children with disabilities, and children with chronic medical conditions.

Noncompliance Area: At the time of the renewal inspection the certification representative observed that the operator did not have verification of submitting the emergency plan to the emergency management agency.

Correction Required: The operator shall send a copy of the emergency plan and subsequent plan updates to the local municipality and to the county emergency management agency.

Noncompliance Area: At the time of the renewal inspection the certification representative observed that the operator did not conduct a fire drill at least once every 60 days.

Correction Required: The operator or designated staff person who is responsible for compliance with this chapter shall conduct fire drills and ensure that fire drills are conducted at least once every 60 days. The operator or designated staff person who is responsible for compliance with this chapter shall conduct fire drills and ensure that a written record is maintained on file at the facility indicating the specific time of day of the drill, the hypothetical location of the fire, the evacuation time, and the names of the facility persons and the number of children who participate in the fire drill.

Noncompliance Area: At the time of the renewal inspection the certification representative observed that the operator did not test and document the testing of the smoke detectors once every 30 days.

Correction Required: The operator or designated staff person who is responsible for compliance with this chapter shall ensure the requirements in § 3290.95(a) are met.
