Noncompliance Area: On 5-21-2026 during a Renewal inspection, the facility did not have an emergency transportation plan.

Correction Required: A written plan shall be conspicuously posted which identifies the means of transporting a child to emergency care and the facility staffing provisions in the event of an emergency. The plan must accompany a staff person who leaves the facility on an excursion with children.

Description: Age and training/Health and Safety Training - Pediatric first aid/CPR

Noncompliance Area: On 5-21-2026 during a Renewal inspection, the files of staff persons #1 and #2 had Pediatric first aid/CPR trainings with an expiration date of 5-16-2026.

Correction Required: Competence is the completion of training by a professional in the field of first-aid and CPR. All staff persons shall renew their certification in pediatric first aid and pediatric cardiopulmonary resuscitation (CPR) on or before the expiration of the most current certification. Staff person shall complete professional development in the topic of pediatric first aid and pediatric cardiopulmonary resuscitation.

Noncompliance Area: On 5-16-2025 during a Renewal inspection, the file of staff person #1 had an expired Mandated Reporter Training.

Correction Required: A facility person's record shall include a copy of requests for the criminal history record and child abuse registry clearance information, a copy of the disclosure statement and a copy of the completed clearance information required under the CPSL. Facility Person #--- may not work in a child care position at the facility.

Correction Required: Fire detection devices or systems must be in compliance with standards established under section 1016(c) of the act (62 P.S. § 1016(c)).

Noncompliance Area: At the time of the renewal inspection the certification representative observed that child #2 did not have a health report on file.

Correction Required: A health report shall include a review of the child's health history.

Description: Immunization record/Religious belief/strong personal objection

Noncompliance Area: At the time of the renewal inspection the certification representative observed that child #1 did not have a record of their immunizations on file. No exemption letters were observed.

Correction Required: A health report shall include a review of the child's immunized status according to recommendations of the ACIP. Exemption from immunization for religious belief or strong personal objection equated to a religious belief shall be documented by a written, signed and dated statement from the child's parent or guardian. The statement shall be kept in the child's record.

Description: Parent written verification from Doctor./Religious belief/strong personal objection

Noncompliance Area: At the time of the renewal inspection the certification representative observed that child #4 has an influenza immunization that was not updated in 2022 or 2023. No exemption letter were observed.

Correction Required: The facility shall require the parent to provide updated written verification from a physician, physician's assistant, CRNP, the Department of Health or a local health department of ongoing vaccines administered to an infant, toddler or preschool child in accordance with the schedule recommended by the ACIP. Exemption from immunization for religious belief or strong personal objection equated to a religious belief shall be documented by a written, signed and dated statement from the child's parent or guardian. The statement shall be kept in the child's record.

Description: General Health and Safety

Noncompliance Area: At the time of the renewal inspection the certification representative observed that the facility did not have a written policy for, "shaken baby and abusive head trauma". The facility had an emergency plan that did not identify the duties of each staff person during an emergency. The facilities emergency plan did not outline the continuity of operations during an emergency. Staff #1, and 2 did not have the health and safety update training completed.

Correction Required: Conditions at the facility may not pose a threat to the health or safety of the children.

Noncompliance Area: At the time of the renewal inspection the certification representative observed that child #5 did not have signed parental consent for emergency medical care.

Correction Required: A child's record must contain signed parental consent for emergency medical care for the child. Written consent is required prior to admission.

Correction Required: The facility shall have an emergency plan that provides for accommodations of infants and toddlers, children with disabilities, and children with chronic medical conditions.

Noncompliance Area: At the time of the renewal inspection the certification representative observed that the operator did not have verification of the review of he emergency plan.

Correction Required: The operator shall review the emergency plan at least annually and update the plan as needed. Each review and update of the emergency plan shall be documented in writing and kept on file at the facility.
