Noncompliance Area: AT THE TIME OF THE RENEWAL INSPECTION THE CERTIFICATION REPRESENTATIVE OBSERVED THAT THE FACILITY DID NOT HAVE SAFE ROUTES POSTED IN THE FACILITY.

Correction Required: Safe pedestrian crossways, pick-up and drop-off points and bike routes shall be appropriately determined in the vicinity of the facility and communicated to the children and parents in writing. Written notification of safe routes shall be posted by the operator at a conspicuous location in the child care facility.

Noncompliance Area: AT THE TIME OF THE RENEWAL INSPECTION THE CERTIFICATION REPRESENTATIVE OBSERVED THAT STAFF #1 DID NOT HAVE A MANDATED REPORTER TRAINING THAT WAS LESS THAN 60 MONTHS OLD. STAFF # 2 DID NOT HAVE A PENNSYLVANIA STATE POLICE CLEARANCE, ONLY THE RECEIPT.

Correction Required: The operator shall comply with the CPSL and with Chapter 3490 (relating to protective services). LACKING REQUIRED HIRING DOCUMENTS: Facility Person # -- may not work in a child care position at the facility.

Noncompliance Area: AT THE TIM EOF THE RENEWAL INSPECTION THE CERTIFICATION REPRESENTATIVE OBSERVED THAT CHILD #2 HAD AN EMERGENCY CONTACT FORM THAT DID NOT HAVE THE MA POLICY # LISTED.

Correction Required: Emergency contact information must include health insurance coverage and policy number for a child under a family policy or Medical Assistance benefits, if applicable.

Noncompliance Area: AT THE TIME OF THE RENEWAL INSPECTION THE CERTIFICATION REPRESENTATIVE OBSERVED THAT CHILD #5 HAD A HEALTH ASSESSMENT ON FILE OLDER THAN 12 MONTHS.

Correction Required: The operator shall require the parent to provide an updated health report at least every 12 months for an older toddler or preschool child.

Noncompliance Area: AT THE TIME OF THE RENEWAL INSPECTION THE CERTIFICATION REPRESENTATIVE OBSERVED THAT STAFF #3 DID NOT HAVE ACCEPTABLE VERIFICATION OF EDUCATION OR PREVIOUS CHILD CARE EXPERIENCE.

Correction Required: A facility person's record shall include verification of child care experience, education and training prior to service at the facility.

Noncompliance Area: AT THE TIME OF THE RENEWAL INSPECTION THE CERTIFICATION REPRESENTATIVE OBSERVED THAT STAFF # 1 HAD A PA STATE POLICE (10/29/13), PA CHILD ABUSE ( 11/19/13), & FBI (11/15/13) CLEARANCE MORE THAN 60 MONTHS OLD AND STAFF #2 HAD A CHILD ABUSE CLEARANCE FOR FOSTER CARE.

Correction Required: The operator shall comply with the CPSL and with Chapter 3490 (relating to protective services). SAFETY PLAN: The operator is required to submit to the Children and Youth Agency a plan of supervision or alternate arrangement as required by the Child Protective Services Law. LACKING REQUIRED HIRING DOCUMENTS: Facility Person # -- may not work in a child care position at the facility. A facility person's record shall include a copy of requests for the criminal history record and child abuse registry clearance information, a copy of the disclosure statement and a copy of the completed clearance information required under the CPSL. Facility Person #--- may not work in a child care position at the facility.

Noncompliance Area: At the time of the inspection no staff member at the facility could provide access to the files of children or staff at the facility.

Correction Required: A staff person shall provide to agents of the Department immediate access to the facility, the children and the files and records

Noncompliance Area: At the time of the follow up to verify corrections from renewal the upstairs rooms was staffed by one individual and there were 7 children including a 2 year old.

Correction Required: When children are grouped in mixed age levels, the age of the youngest child in the group determines the staff:child ratio and maximum group size in accordance with the requirements in 3270.51 (relating to similar age level). The legal entity must submit documentation in a format approved by DHS proving that the facility is maintaining the required staff:child ratios at all times. The legal entity will confirm continuing compliance by sending copies of the documentation to the Southeast Region Office on a weekly basis for a period of three months after DHS approves the documentation format.

Noncompliance Area: At the time of the inspection the facility had two staff persons and a total of 13 children including one infant.

Correction Required: When children are grouped in mixed age levels, the age of the youngest child in the group determines the staff:child ratio and maximum group size in accordance with the requirements in 3270.51 (relating to similar age level).

Noncompliance Area: At the time of the inspection the facility had a broken fence between the stareet and the outdoor playspace.

Correction Required: If unsafe areas or conditions are in or near an outdoor play space, fencing or natural barriers are required to restrict children from those unsafe areas or conditions.
