Noncompliance Area: During the renewal inspection on 1/29/2025, it was observed that 3 electrical outlets upstairs in the front classroom did not have protective covers on them.

Correction Required: Protective receptacle covers shall be placed in electrical outlets accessible to children 5 years of age or younger.

Noncompliance Area: During the renewal inspection on 1/29/2025, it was observed that there was chipping plaster and paint on the purple wall on the first floor.

Correction Required: Peeled or damaged paint or damaged plaster is not permitted on indoor or outdoor surfaces in the child care facility.

Noncompliance Area: During the renewal inspection on 04/03/2023, the health report for child #2 was not signed by a physician, physician's assistant or a CRNP.

Noncompliance Area: During the renewal inspection on 04/03/23, staff person #1, staff person #2, and staff person #3 did not have verification of the annual minimum of 12 clock hours of childcare training.

Correction Required: A staff person shall obtain an annual minimum of 12 clock hours of child care training.

Noncompliance Area: Child # 1, an infant who is 2 and 1/2 months old, was observed sleeping in an infant chair and not flat on his back in a crib or pack n play. While the parent requested that equipment, according to the AAP, neither the infant seat nor the position is age-appropriate or recommended and cannot be used in a licensed child care facility.

Correction Required: Individual, clean, age-appropriate rest equipment shall be provided for preschool, toddler and infant children as agreed between the child's parent and the operator. The rest equipment shall be labeled for the use of a specific child and used only by the specified child. Infants shall be placed in the sleeping position recommended by the American Academy of Pediatrics unless there is a medical reason an infant should not sleep in this position. The medical reason shall be documented in a statement signed by a physician, physician's assistant or CRNP and placed in the child's record at the facility.

Noncompliance Area: Child # 4 was missing physician's information.

Correction Required: Emergency contact information must include the name, address and telephone number of the child's physician or source of medical care.

Noncompliance Area: Child # 4 was missing parent's home and work address and work phone number.

Noncompliance Area: Child # 1, an infant who is 2 and 1/2 months old, was observed sitting in an infant seat, feeding from a bottle propped up on a folded blanket. The child was not being held.

Correction Required: An infant 6 months of age or younger shall be held while being bottle fed.

Noncompliance Area: Facility has siblings enrolled and their records are combined. They do not have separate files, emergency contact forms, or agreements.

Correction Required: An operator shall establish and maintain an individual record for each child enrolled in the facility.

Noncompliance Area: Facility had a TV sitting on a low child's chair and it was not secured. It was wobbly and could be tipped over.

Correction Required: Furniture must be durable, safe, easily cleaned and appropriate for the child's size, age and special needs.

Noncompliance Area: Children # 1, 2, & 3 did not have complete required release person info.

Correction Required: Emergency contact information must include the name, address and telephone number of the individual designated by the parent to whom the child may be released.

Noncompliance Area: The written plan of daily activities was not posted in first floor child care space.

Correction Required: The written plan of daily activities shall be posted in the group space. Post the written plan of daily activities in each child care space.

Noncompliance Area: One child's service reports were not being completed every 6 months in accordance with these regulations. (#2)

Correction Required: The services to be provided to the family and the child, including the Department's approved form to provide information to the family about the child's growth and development in the context of the services being provided. The operator shall complete and update the form and provide a copy to the family in accordance with the updates regarding emergency contact information in 3270.124(f) (relating to emergency contact information). Revise and submit written policies and procedures regarding the completion of child service reports every 6 months to ensure ongoing compliance.

Noncompliance Area: One staff person was observed changing a child's diaper and did not ensure that the child's hands were washed after being diapered.

Correction Required: A staff person shall ensure that a child's hands are washed before meals and snacks, after toileting and after being diapered. The legal entity must arrange for all staff persons to be trained in proper hand washing procedures. Submit written verification of the completed training for all facility staff.

Noncompliance Area: One child's record did not contain financial agreement information that had been reviewed and updated within the past 6 months.

Correction Required: A parent is required to review and update the emergency contact information and the financial agreement at least once in a 6-month period or as soon as there is a change in the information. Obtain and submit written verification of the updated financial agreement information for the identified child. Revise and submit written policies and procedures to ensure that children's emergency contact information is updated at least every 6 months or as soon as the information changes.

Noncompliance Area: One child's record did not contain signed parental consent for emergency medical care and for administration of minor first-aid procedures by facility staff. (#2)

Noncompliance Area: The sink in the bathroom used by children were not at a proper height for the young toddlers and the step stool was not at a sufficient height to make it easily approachable.

Correction Required: Toilets and sinks shall be at proper heights for children using them or shall be easily approached by means of platforms or steps. Obtain a platform or steps at an appropriate height to make the sink more approachable for the young toddlers.

Noncompliance Area: Handwashing signs posted in the bathrooms did not specify that a facility person and an able child shall wash his hands after toileting and before eating.

Correction Required: A facility person and an able child shall wash his hands after toileting and before eating. A sign on which this requirement is written shall be posted at each toilet, training chair, diapering area and sink in the facility. Revise and post hand washing signs that includes the information as specified in this regulation.

Noncompliance Area: Two childrens emergency contact information did not include the names of the individual designated by the parent to whom the child may be released; they only stated mom or dad. (#5)

Correction Required: Emergency contact information must include the name, address and telephone number of the individual designated by the parent to whom the child may be released. Obtain and submit up dated emergency information that includes the full names of the individuals to whom the child can be released. Ensure that all childrens records contain emergency contact information as specified in these regulations.

Noncompliance Area: One child's record contained a health report completed more than 60 days (6 months) following the first day of attendance at the facility. (#2)

Correction Required: The operator shall require the parent of an enrolled child, including a child, a foster child and a relative of an operator or a facility person, to provide an initial health report no later than 60 days following the first day of attendance at the facility. Revise written policies and procedures to ensure that childrens initial health reports are completed and provided as specified in this regulations. Submit a written copy of the revised policies and procedures.

Noncompliance Area: Three childrens records did not contain a health report completed within the past 12 months. (#1, #3 and #5)

Correction Required: The operator shall require the parent to provide an updated health report at least every 12 months for an older toddler or preschool child. Obtain and submit a current up-to-date health report for the indicated children. Ensure that all childrens records include a current up-to-date health report as specified in this regulation.

Noncompliance Area: Emergency contact and financial agreement information in four childrens records were not being updated every 6 months.(#1, #2, #3, #4 and #5).

Correction Required: A parent is required to review and update the emergency contact information and the financial agreement at least once in a 6-month period or as soon as there is a change in the information. Revise and submit written policies and procedures to ensure that childrens emergency contact information is updated at least every 6 months or as soon as the information changes. Obtain and submit current up-to date financial agreement information for the identified child.

Noncompliance Area: Several children's records did not contain signed parental consent for emergency medical care and for administration of minor first-aid procedures by facility staff. Some had initials rather than the parents signature indicating consent others contained no signatures. (#2, #3 amd #5)

Correction Required: A child`s record shall contain signed parental consent for emergency medical care for the child. Noncompliance Area: The first aid kit in two classrooms did not contain all of the required contents; gloves, bandages, soap scissors and tweezers.

Correction Required: A first-aid kit must contain the following: soap, an assortment of adhesive bandages, sterile gauze pads, tweezers, tape, scissors and disposable, nonporous gloves. Replenish the first aid kit and ensure that they contain all of the required contents as specified in this regulation.
