I used to work here was not happy they would make child sit in seats wiht seat belt at the end of day sometimes for more than a hr would not let employees use restrooms when needed you was not alloed to sit down for any reason did not feel happy there at all did not feel like a family They were very good to the children mostly but i did not feel happy

Center staff did not comply with the safe sleep policy. In space #2, the posted safe sleep policy specified that caregivers shall visually check, in person, sleeping infants aged 12 months or younger at least every 15 minutes. On 1/10/2024, one child's visual checks were documented at 11:40am and not again until 12:00pm. On 1/11/2024, 1/12/2024, and 1/16/2024, one child's visual checks were documented at 11:25am and not again until 11:45am. On 1/15/2024, one child's visual checks were documented at 11:20am and not again until 11:45am.

Daily records of arrival and departure times for children enrolled at the center were not maintained as children arrive and depart and/or were not made available for review. Two children were not signed out on May 15, 2023. One child was not signed out on May 9, 2023. One child was not signed out on May 8, 2023.

All stationary equipment, more than 18 inches high, was not installed over protective surfacing. On the playground for preschool children, several fall zone areas surrounding the large composite structure only contained a scattering of mulch, including the base of both green metal pole climbers, the sides of the blue, single slide, and the sides of the green, double slide. The existing mulch has compacted into a solid dirt mixture.

Center staff did not comply with the safe sleep policy. In space #1B, staff did not follow safe sleep policy item #13 stating "we do not allow any objects other than pacifiers such as pillows, blankets, or toys in the crib or sleep space." One infants crib contained four toys and a Boppy pillow hanging down from the inside of the crib rails into the sleep space where an infant was observed asleep in the crib.

The child care operator did not comply with all State laws, federal laws and/or local ordinances that pertain to child health, safety, and welfare, as required by General Statute 110-91. Sand toys, water tables and other items that can collect standing water in the outdoor learning environment shall be emptied and stored to prevent standing water. The turtle box with the intended use of a sand box was observed on the playground containing two inches of water. This is a violation of sanitation requirement 15A NCAC 18A .2832(b).

Emergency and identifying information, including the child's name, photograph, emergency contact information and/or a copy of the emergency medical care information form was not in the vehicle for each child being transported. Emergency and identifying information was not on the vehicle for two school-age children transported to the facility on May 18, 2023.

Electrical outlets and power strips, not in use, which were located in space used by children did not have safety outlets or were not covered with safety plugs unless located behind furniture or equipment that cannot be moved by a child. In space #2 and space #3, the electrical outlets at the end of the Christmas strand lights were uncovered and accessible to children. The Administrator covered the electrical outlets not in use during the visit.

Emergency information record did not include chronic illness and any medication taken for the illness. The application for one child enrolled on June 4, 2018 did not include that the child had a Medical Action Plan nor did the emergency information include that the child had an Epi-Pen.

A current activity plan was not posted for each group of children for reference. In space #4, the activity plan posted in the classroom was dated May 16, 2022 through May 20, 2022. Staff corrected this during the visit by posting the current activity plan for the week of May 30, 2022.

The written feeding plan was not modified as the child's needs changed. Three children's Infant Feeding Plans were updated to reflect table food could be given but was not completed in conjunction with the parent. The parents did not initial the update on the Infant Feeding Plan as required.

All stationary equipment, more than 18 inches high, was not installed over protective surfacing. On the playground for preschool children, the mulch in the fall zone area under and around the stationary play structure only contained a ground covering of compacted mulch rather than the required six inches in depth.
