Center staff did not comply with the safe sleep policy. In space #2L, one infant was observed asleep in a crib on his stomach at 2:55pm and the initial sleep position and the time the infant was laid in the crib was not recorded on the safe sleep visual check chart. Staff stated the infant fell asleep at 2:45pm. Documentation was not available to confirm whether the child was placed on their back to sleep.

Operator did not submit the original approved report to DCDEE within one week of the inspection visit on a form provided by the Division. In space #3L, three bottles of essential oils with multiple safety warnings were observed in an unlocked cabinet accessible to children. The Co-Director removed the essential oils from the classroom to locked storage during the visit.

One staff employed on 2/28/2022, did not complete the annual Emergency Information document until 3/6/2023.
