A copy of the Notice of Inspection Rights was provided at the time of the inspection.

Initial Comments: No deficiencies were observed at the Modification Inspection conducted on 3/5/26, and is subject to changes pending programmatic review. An infant room was added at this time. No infants were present at the time of the inspection. A copy of the Notice of Inspection Rights was provided at the time of the inspection. The following was discussed but not limited to: 1) The 2 miniature boppy/neck pillows were removed from the infant room and the licensee and director were reminded that all soft products, including boppy pillows and stuffed animals, are prohibited from being used in the tummy time space per R9-5-502.A, 2) The 2 walkers were removed from the infant room since R9-502.B.3 prohibits the use of walkers in the infant room, 3) The 4 Lakeshore leaf pillows were removed since the manufacturer's specifications are for children 18 months and older, 4) Tummy time rule requirements per R9-5-502.A, 5) Written Infant feeding instructions requirements per R9-5-502.C, and 6) Crib spacing when cribs are occupied.

Initial Comments: The following deficiencies were found at the time of the Compliance Inspection conducted on 6/26/2025 and are subject to changes pending programmatic review. The Written Documentation is required to be submitted through the Licensing Portal within 10 days of the receipt of the Statement of Deficiencies. The following was discussed but not limited to: 1) Ensuring all staff submit DCS clearance applications through the DCS portal that was effective August 2024, and 2) License Anniversary fee due by 8/31/25. 3) Listing the symptoms of a child’s antiphallic reaction on the child's medication form for an EpiPen, so the licensee knows when the EpiPen is required to be administered, and 4) Date of employment versus starting date, as related to timeframes for obtaining fingerprint clearance card, TB test, DCS background clearance etc. ** Please submit a copy of the insurance certificate and City of Scottsdale fire inspection, since both will renew in July 2025. There were 4 staff files reviewed. 4 of the 4 fingerprint clearance cards were verified to be valid through the DPS website.

Initial Comments: AMENDED The following deficiencies were observed at the time of the Compliance Inspection conducted on 7/25/2023 and are subject to changes pending programmatic review. A plan of correction is not being accepted at this time. The 2 outdoor activity areas were not inspected. Please email pictures of the outdoor activity areas when constructed is completed and are ready to be used. An inspection of the 2 outdoor activity areas MUST be conducted prior to being used by enrolled children. The vehicle was not inspected. The facility will schedule an appointment to have the vehicle inspected prior to being used by enrolled children. 4 of 4 Fingerprint Clearance cards reviewed were valid via a DPS website search. The Empower self-evaluation was completed at the time of the inspection. The Emergency Disaster Plan update form was completed at the time of the inspection. The DES Group size checklist was completed at the time of the inspection. The following was discussed but not limited to: 1. All staff must document arrival and departure times. 2. I have toured this provider's facility, but have not used its services

After speaking with a parent recently I discovered he called and warned the staff that his daughter had been diagnosed that day with "Hand Foot Mouth Disease" which is extremely contagious. The preschool proceeded to allow the childs mother to drop her off there the very following day and exposed all of the children to the disease. I am appalled and will not be using this facility.

Our center prides itself on providing children a safe , enjoyable, and clean atmosphere. Per policy, parents are to provide a doctor's note informing center of child's condition upon returning. Once informed of any illness, we diligently assess all scenarios. This policy is enforced in order to provide all children and families the most valuable experience while providing care.

my name is callyn and when i went to little exploers everyone was mean, they told us scary stories, made us sit behind a big peice of wood for time out, and then there was the dreaded day my broter was litteraly scared for life, me and my brother Isaac have always stuck together like paper and glue. But we knew that when it came time to go to preschool we had to seperate,(becuasewe were different ages.) so wheile im in the other room one of the teachers told ANOTHER STUDENT TO PUSH MY BROTHERINTO THE TABLE, and as my mom is rushing over there my brother is BLEEDING FROM RIGHT ABOVE THE EYE, SCARED OUT OF MY OWN MIND THEY WONT LET ME IN, iam twelve now and hoping that NO OTHER CHILD GOSE THROUGH WHAT MY BROTHER AND I WENT THROUGH THERE.
