Please submit the Plan of Corrections via the LMS portal within 10 days of receipt of the Statement of Deficiencies. 5 of 5 Fingerprint Clearance Cards reviewed were valid via the DPS website. The following was discussed, but not limited to: The DCS portal that was effective in July 2024, that all current staff are required to be entered into.

Initial Comments: The following deficiencies were found at the time of the Compliance Inspection conducted on 8/1/2024, and are subject to changes pending programmatic review. Compliance Officer #1: Jennifer Flicker Compliance Officer #2: Chloe-James Rossi The Plan of Corrections is required to be submitted through the Licensing Portal within 10 days of the receipt of the Statement of Deficiencies. The following was discussed, but is not limited to: 1) Diapering sink temperature range, 2) Anniversary fee due by 8/31/2024, 3) Application required for modification of age to 1-year-old in room 107 4) Due to reasons of sanitation, items stored in a bathroom must be stored inaccessible to enrolled children and in either a closed cabinet or in a waterproof container covered with a lid. There were 6 staff files reviewed. 6 of the 6 fingerprint clearance cards were verified to be valid through the DPS website.

Initial Comments: The following deficiencies were observed at the time of the Compliance Inspection conducted on 8/10/2023 and are subject to changes pending programmatic review. Please submit the Plan of Corrections via the LMS portal within 10 days. 7 of 7 Fingerprint Clearance cards reviewed were valid via a DPS website search. The Empower self-evaluation was completed at the time of the inspection. The Emergency Disaster Plan update form was completed at the time of the inspection. The following was discussed but not limited to: 1. Peeling paint near the low clear room divider doors. 2. Peeling netting on the playground fence.
