|Findings: Staff #2 (start date 7-25-16) fingerprint clearance card was received at DPS on 8-10-16. Staff #2's Level One Clearance card was issued 9-2-16.|
|Findings: Preschool Program: The file for staff #2 (date of employment- 8/5/2016) contained documentation indicating that the staff's Mantoux skin test was read on 8/10/2016. It was determined that the staff did not submit documentation of the staff's evidence of freedom from infectious active tuberculosis on or before the staff's starting date of employment.|
|Findings: During a review of the Kidsclub Director's file (staff #3), the Surveyor determined that the file contained documentation of the staff's Bachelor's degree in Business but contained documentation of only 24 of the 60 required hours of instruction, provided in conferences, seminars, lectures, or workshops in early childhood, child development, or a closely-related field and lacked documentation of an additional 12 hours of instruction, provided in conferences, seminars, lectures, or workshops in the area of program administration, planning, development, or management.|
|Findings: Preschool Program: The file for staff #1 (date of employment- 1/12/2015) contained documentation of training and indicated that the staff had completed only 13.5 of the required 18 hours of annual training for the period of 8/10/2015 through 8/10/2016.|
|Findings: During an inspection of the outdoor activity area, the Surveyor observed that the sand measured less than 2 of the required 6 inches at the base of the orange slide and under the climbing structure.|
|Findings: During a review of staff files, it was determined that the Central Registry Background Clearance letters were kept at the District Office and were not kept on site.|
|Findings: Kidsclub: 1)The card for child #1 was not signed by a parent; and 2)The card for child #2 did not include the telephone number of the child's health care provider.|
