|Findings: The Surveyor requested the staff attendance records for February 15, 2018 to review. Staff stated that they had not recorded their time of arrival for February 15, 2018.|
|Findings: Staff #3 (start date 8-10-17) negative Mantoux skin test was administered on 1/31/18, after Staff #3's stating date of employment.|
|Findings: Staff #1 stated that Staff #2 (start date 9-16-13) written documentation of annual training for 9/2016 - 9/2017 will be placed in the file.|
|Findings: During a review of 2 staff files, the Surveyor determined the files for staff #1 and #2 lacked documentation of the licensee's submission of the staff's Central Registry Background Clearance check and of the staff's Central Registry Background Clearances.|
|Findings: During an inspection of the preschool room, the Surveyor observed that 8 of the 18 mats that children were laying on did not have an unobstructed passageway of at least 18 inches wide that would allow a staff member to access the children. These mats were obstructed either by furniture or the pathway was less than 18 inches.|
|Findings: Finding: During an inspection of the first aid kit, the Surveyor determined that the first aid kit did not include the 2 required rolls of gauze and contained only one gauze pad.|
|Findings: Finding: During a review of the Accident and Emergency plan, the Surveyor observed that the plan was dated 8/2015 and it could not be verified that the plan had been updated within the last 12 months.|
|Findings: During a discussion with the Preschool Program Manager, the Manager acknowledged that the staff's Central Registry Background Clearance letters were kept at the District office and were not kept on site.|
|Findings: The Licensee requested that Room 107 be self-limited to a capacity of 18 when the room was approved by the Department in August 2015. The staff told the Surveyor that 19 children are now enrolled and attend the program.|
|Findings: Staff #1- date of employment- 8/8/2011. The file contained documentation indicating that the staff's Mantoux skin test was read on 1/27/2014. It was determined that the staff did not submit documentation of the staff's evidence of freedom from infectious active tuberculosis on or before the staff's starting date of employment.|
