Initial Comments: The following deficiencies were found at the time of the Compliance Inspection conducted on 1/29/2026, and are subject to changes pending programmatic review. The Plan of Corrections is required to be submitted through the Licensing Portal within 10 days of the receipt of the Statement of Deficiencies. The following was discussed but is not limited to: *Ensure napping mats are maintained free from hazards, and *Ensure staff Criminal Affidavits are complete. There were 2 staff files reviewed. 2 of the 2 fingerprint clearance cards were verified to be valid through the DPS website. The Compliance Officer provided a paper copy of the Notice of Inspection Rights and the Small Business Bill of Rights to the Facility Director at the time of the inspection.

Initial Comments: The following deficiencies were observed at the time of the Compliance Inspection conducted on 2/4/2025 and are subject to changes pending programmatic review. Please submit the Plan of Corrections via the LMS portal within 10 days of receipt of the Statement of Deficiencies. The Compliance Officer provided a paper copy of the Notice of Inspection Rights to the provider at the time of inspection. The Empower Self Assessment was emailed to the provider. The Emergency Disaster Contact Form was provided to the provider. Please complete and return within 10 days of the inspection. The fingerprint clearance cards for 4 of 4 staff members were verified to be valid through the DPS website at the time of the inspection. The following was discussed but not limited to: 1). Ensuring that lesson plans include the amount of minutes children are having screen time. 2). Ensuring that items stored in the bathroom are in a waterproof container. 3).
