| Prior to the expiration date of the qualification letter, the child care provider did not complete and submit required forms to complete a criminal background check (a qualification letter is valid for a maximum of three years for the date of issuance). A CBC status was located in Regulatory but it was not current. The staff member (SB) stated that she was not aware after completing the live scan that the process was not completed. | |||
