| Openings in equipment, steps, decks, handrails, and fencing were not less than 3 1/2 inches or greater than 9 inches. The larger playground’s equipment was in good repair but the maintenance staff was in the middle of upgrading the bottom of the fence to be in compliance. While the depth of the bottom of the fence has openings that are not less than three and one half (3 and ½) inches or greater than nine (9) inches There were also protrusions with rugged and pointed end that could be a hazard. While most of the playground is finished and in compliance, there is an area near the gate which is out of compliance. | |||

| Children were not adequately supervised at all times. The Administrator, having self-reported the incident, stated that on September 27,2022 whereas a child was left in a classroom for what was reported to be less than a minute. She stated that he was in line before they left the classroom to go to lunch and then when the class got to the cafeteria, they determined that the child was not present and went back to the classroom to locate the child. The child was in the floor playing. The child was then taken to the cafeteria where he rejoined his class. This violation was corrected immediately. | |||
