A drug or medicine was administered after its expiration date. In space #2a, I observed one (1) expired diaper cream. I observed an EPI pen that belongs in space #3a/b with an expiration date of 12/2022.

The indoor and/or outdoor premises was not checked once a day, prior to initial use, ensuring debris, and broken equipment was removed and disposed of. I observed trash debris and trash piles in both playgrounds.

I have toured this provider's facility, but have not used its services

Everyone was friendly and welcoming as I toured the center. They answered all my questions promptly. I am pleased to enroll my child
