When three year old children and older were in care, the materials and equipment in the activity area was not in sufficient quantity to allow at least three children to use the area regardless of whether the children choose the same or different activities. In both of the classrooms there was only two different types of items in the Book/Library Center: Books and magnets with one magnet board in Space 3 and Books and Puppets in Space 2. There were not the sufficient materials for three different activities to play with different activities.

Of the two files that were reviewed today one staff person doesn't have any record of the Orientation trainings completed and the other had training that was required within the first two (2) weeks, six hours in the first five (5) topics, where it took four (4) weeks to complete.

The Prevention of Shaken Baby Syndrome and Abusive Head Trauma policy was not reviewed with new staff prior to providing care with children and/or a signed acknowledgement with all the required information was not maintained in the staff person's file. Of the two (2) staff files that were reviewed today none of the files had a signed acknowledgement that the policy had been reviewed with them.

All equipment and furnishings were not in good repair. On the playground for preschool children the following items were found: one broken plastic 'milk crate' storage bin; peeling paint on the yellow metal 'school bus'; one tricycle missing both pedals.

Electrical outlets and power strips, not in use, which were located in space used by children did not have safety outlets or were not covered with safety plugs unless located behind furniture or equipment that cannot be moved by a child. In space #5 there was one electrical outlet uncovered and accessible to children.
