| Initial Comments: The following deficiencies were observed at the time of the Compliance Inspection conducted on 8/6/2024 and are subject to changes pending programmatic review. Please submit the Plan of Corrections via the LMS portal within 10 days. 5 of 5 Fingerprint Clearance cards reviewed were valid via the DPS website search. The Empower self-evaluation was completed at the time of the inspection. The Emergency Disaster Plan update form was completed at the time of the inspection. The following was discussed but not limited to: 1. Posting of refunds. 2. Enrollment policies in the Statement of Services/ Parent Handbook. 3. Sterile gauze rolls in the first aid kit. 4. All staff must record times of arrival and times of departure. 5. Children's sign in and sign out records with times and signatures. 6. Staff files must include 2 good faith effort references and letters of enrollment in school for the student caregivers. 7. Evacuation maps must show the route to use to evacuate. 8. Spider webs on the outdoor climbing structure. 9. Cleaning the tables in the outdoor environment prior to being used by enrolled children. 10. Record cards must contain dates of enrollment. 11.
