The Emergency Disaster Contact form was completed during the inspection. BCCL staff emailed the Empower Self-Evaluation Assessment link to the Provider. 3 of 3 fingerprint clearance cards were verified as valid via the DPS website during the inspection. | Initial Comments: The following deficiencies were found at the time of the Compliance Inspection conducted on 5/2/2025, and are subject to changes pending programmatic review. The Plan of Corrections is required to be submitted through the Licensing Portal within 10 days of the receipt of the Statement of Deficiencies. The following was discussed, but is not limited to: *Ensure 6 inches of resilient surfacing is maintained in the rock wall fall zones, *Ensure Kinder Prep information is posted on the main posting board, *Ensure children younger than 5 years old do not use the play structure, and *Ensure staff files are complete. There were 3 staff files reviewed. 3 of the 3 fingerprint clearance cards were verified to be valid through the DPS website.
