Initial Comments: There were no deficiencies observed at the time of the Initial Monitoring Inspection conducted on 12/10/2025. The Compliance Officer provided the facility with a paper copy of the Notice of Inspection Rights at the start of the inspection. The DES Contact Group Size was in compliance at the time of the inspection. The fingerprint clearance cards for 5 of 5 staff members were verified to be valid through the DPS website at the time of the inspection.

Initial Comments: There were no deficiencies observed at the time of the Initial Inspection conducted on 10/21/2025. The Compliance Officer provided the facility with a paper copy of the Notice of Inspection Rights at the start of the inspection. The DES Contact Group size was in compliance at the time of the inspection. The fingerprint clearance cards for 10 of 10 staff members were verified to be valid through the DPS website at the time of the inspection. During the exit interview, the following items were discussed, but not limited to: The new rule set and the location of all new documents. The fall zones have at least 6" of woodchips. (continuously move to the fall zones) Children’s sign in and out records-unique identifier Children's Emergency Information and Immunization records Staff attendance records Nap time Available drinking water to means the needs of the children Monthly fire drills Classroom rosters Notification of pesticides Child abuse and neglect- mandated reporter Children with special needs and in the individualized plan Discipline and guidance procedures Written accident, evacuation and emergency plans Notifying parents and documenting injuries Illness and infestation log Medication procedures Transportation and Field Trip Procedures Notify the Department of closure for any days Interview rights of staff and children
