The Compliance Officer provided a paper copy of the Notice of Inspection Rights to the facility director at the time of the inspection. A link to the Empower Self-Evaluation Survey was sent to the facility director. The Emergency Disaster Plan update form was completed at the time of the inspection. Four fingerprint clearance cards reviewed were verified as valid through the DPS website. Please submit the Plan of Corrections via LMS portal within 10 days of receipt of the Statement of Deficiencies.
