|Findings: When the Surveyor, the Director and the Assistant Director were passing through room #6, the Surveyor observed 2 children entering the toilet room that is located in the room. Room #6 was not in use at that time. The Director stated that the children came from room #2 to use the toilet room. The ratio observed by the Surveyor was 0:2.|
