| Initial Comments: The following deficiencies were observed at the Compliance Inspection conducted on 3/19/26, and are subject to changes pending programmatic review. The Plan of Corrections is requested to be submitted in the LMS portal within 10 days of receipt. The following was discussed but not limited to: 1) All current staff are required to submit an application for DCS clearance through the DCS portal that was effective July 2024, and 2) New rules effective 8/2025. Please send the following: 1) City of Phoenix Fire permit, when obtained, and 2) DCS clearance for cited staff. A copy of the Notice of Inspection Rights was provided at the time of the inspection. BCCL staff emailed the Empower Self-Evaluation Assessment link to the Provider. 4 of 4 fingerprint clearance cards were verified to be valid through the DPS website during the time of the inspection. | |||

| Initial Comments: The following deficiencies were found at the time of the Compliance Inspection conducted on 4/10/2025 and are subject to changes pending programmatic review. The Written Documentation is required to be submitted through the Licensing Portal within 10 days of the receipt of the Statement of Deficiencies. The following was discussed but not limited to: 1) Repairing the beginning stages of cracking on the outdoor playground rubberized surfacing, 2) Rosters require the last name of the children who are listed, 3) Licensee requests to self limiting the capacity of each room to 20, 4) Noting the symptoms on the child’s medication permission form that manifest for the child, so staff know when to administer the child’s Epipen, and 5) Effective August 2024, all Providers are required to use the DCS portal for obtaining staff’s DCS clearance. Please send the DCS clearances for staff #1-4 to the Compliance Officer, when obtained.
