| Initial Comments: The following deficiencies were observed at the Initial Inspection conducted on 3/23/26, and are subject to changes pending programmatic review. The Plan of Corrections is requested to be submitted in the LMS portal within 10 days of receipt. A copy of the Notice of Inspection Rights was provided at the time of the inspection. The following was discussed but not limited to: 1) The diaper changing areas for the two 2-year-old rooms are required to be relocated to inside the classroom by the initial monitoring inspection. The attached drinking fountain may not be used with the proposed diapering sinks, and 2) Ensuring accuracy and validity of staff file documentation, including documentation of education. 13 of 13 fingerprint clearance cards were verified to be valid through the DPS website during the time of the inspection. | |||
