Findings: Based on facility documentation and the Surveyors' observations, it was determined that the roster in the following classrooms did not reflect the actual number of children present: 1's classroom: There was no roster to reflect the presence of the 5 children in the classroom. 3's classroom: There were 8 children present in the classroom but the roster reflected the presence of only 2 children in the classroom.

Findings: Based on Surveyor #1's observation and review of facility documentation, it was determined that Staff #2 was not qualified to be a teacher-caregiver. There was no documentation showing that Staff #2's education from a foreign country had been evaluated to reflect that it was equivalent to at least a US High School Diploma.

Findings: Based on the Surveyors' observations, it was determined that there were two yellow blocks, one red coffee cup, one blue squishy whale and three blue bibs in the diaper handwashing sink in the 3's classroom.

Findings: Based on the Surveyors' observations, it was determined that in the 3's classroom, the water temperature in the hand-washing sink next to the diaper changing surface was 120 F instead of the required temperature range of 86 F and 110 F.

Findings: Based on the Surveyors' observations, it was determined that the facility lacked an outdoor activity area for the enrolled children to use. Upon arrival at the facility the Surveyors observed that the playground was under construction and the children did not have any access to the outdoor activity area. The Director stated that the playground will be closed from 5/3/2021 until 5/14/2021 for remodeling.
