Findings: Based on Surveyor #1's observation and staff statement, in the 3's room, the staff to child ratio was 1:14, with three-year-old children present in the room, and a required ratio of 2:14, when Staff #2 left the room and walked down the hall to ask the director if it was time for her to go on a break. Staff #2 stated she left the room to see if she could go on a break and left the other staff alone with 14 children.

Findings: Based on Surveyor #1's observations, the health, safety and welfare of enrolled children was placed at risk of harm when, during naptime, blankets were covering 7 enrolled children's faces/heads in 3 of 4 classrooms. The staff were not able to observe the sleeping children's faces.

Findings: Based on Surveyor #1's observations and staff interviews, there was no drinking water available for the 6 one-year-old children in the Toddler 1 room. A staff stated the sippy cups are in the cabinet. (The water fountain in the room drizzled water down the spigot.) There was no drinking water available for the 11 two-year-old children in the Toddler 2 room. A staff stated the cups are kept in a cabinet. (The water fountain in the room drizzled water down the spigot.) Staff stated when the three-year-old children are on the playground, she brings the children in to get them a drink of water. (The fountains on the playground have been turned off.) There were 27 school-age children present on the playground and 3 children had water bottles. The 2 water fountains had been turned off. Staff stated they take the children inside to get a drink of water and some of the children bring water bottles.

Findings: Based on Surveyor #1's observations, the following hazards were accessible to enrolled children: Toddler 2: There was exposed particle board on a toy shelf. School-age room: The light was very faint in the boys' restroom, there were 2 areas of exposed foam on a blue bar stool, and there was a 4 foot long black charging cord on the floor near a table. In the school-age restroom, the toilet seat was loose and moved easily when touched.

In the Toddler 1 room, there was one container of wipes labeled with a child's first name and last initial only. Findings: Based on facility documentation, Surveyor #1's observations and staff statements, the written feeding instructions for Child #3 was not current. The plan was dated 2/24/2021 and did not include the foods to be fed to the child. Staff stated the child eats everything and the plan is not accurate. The facility lacked written feeding instructions for Child #4. Staff stated the child just started yesterday.
