School-age classroom: Paint on the wall was peeling by the large rug. The wood door to the bathroom had 15 holes exposing unsealed wood. Preschool Classroom: Paint on the wall was peeling exposing drywall by the sinks and by the workbench. The bathroom wall had three holes next to the paper towel dispenser.

Two's classroom: The shelf with musical instruments had a quarter size area of peeling laminate, creating rough edges and unsealed wood on the top panel. Preschool classroom: The red seat on the tricycle was cracked creating a rough edge. Paint was peeling off the dramatic play kitchen.

Preschool playground: * There was areas of chipped paint, exposing rust and creating sharp edges on the base of the perimeter fence. * The rubber on the large climbing structure was peeling on the steps and edges of the platforms. exposing rusty metal. *The yellow and blue square seats had a build-up of pigeon feces.
